Academic Progress Disclaimer
Participation in tutoring services provided by Dr. S’ Learning Institute (DSLI) is intended to support and enhance a student’s academic development. While instructional strategies and evidence-based practices are used to support learning, Dr. S’ Learning Institute does not guarantee specific academic outcomes, including but not limited to grade improvements, test scores, or school placement. Academic success depends on many factors such as student effort, attendance, home support, and school instruction.
By enrolling in services, parents/guardians acknowledge that tutoring is supplemental educational support and agree that Dr. S’ Learning Institute cannot be held responsible for a student’s academic performance in school.
Media and Photo Release
I grant permission for Dr. S’ Learning Institute to photograph or record my child during tutoring sessions, workshops, or educational activities for the purpose of educational promotion and marketing. These images or recordings may be used on the organization’s website, social media pages, or promotional materials.
I understand that Dr. S’ Learning Institute will not include identifying personal information such as last names, addresses, or school information without additional consent.
If I do not wish for my child to appear in photos or videos, I will notify Dr. S’ Learning Institute in writing, and every reasonable effort will be made to honor this request.
Parents/guardians understand that tutoring services are educational in nature and that Dr. S’ Learning Institute is not responsible for supervising students outside of scheduled session times.
